We intend to provide support for people to promote independence. We recognise that prospective Service Users should have the opportunity to choose who works with them to provide their support needs.

The Range of Support

The support being offered aims to help our service users to develop skills, on a person-centred basis, giving the service user confidence and vision they need to live more independently and integrate safely in the local community.

The ways in which we support service users may include providing advice, prompting or assistance with the following

Mental Health

Vivifica Healthcare Limited aims to support vulnerable people who suffer from mental health needs, such as anxiety disorder, bipolar disorder, depression, dementia and physical disability supporting them to live a stable and independent life, maintaining their dignity and promoting their independence.

Vivifica Healthcare Limited aims to address some of the challenges individuals with mental illness, face whilst trying to integrate back into the community. Some of the challenges are the quality-of-care people receive, limited access to services and relevant support, lack of access and accessibility to mental health community services, social isolation, inadequate support from local authorities with regards to care packages, including general advice and support.

Accessibility to Care

We will ensure that service users have accessibility to care in a timely manner and referrals made when their care needs change; for example, in the event of a service user transitioning to more complex care needs, a multiagency approach will be used whereby, we will seek advice and make referrals according to the service users changing needs. Care plans and risk assessments will be updated accordingly and will be person-centered with sufficient detail, to ensure transparency and clarification for staff delivering the care. Training will be ongoing and updated as part of this process to ensure staff are always equipped with suitable skills to carry out the required care. Additional training will be sourced to allow the service user to live in their own home for as long as it is safe to do so.

Risk Assessments

We will promote positive risk taking and will carry out individual risk assessments, which will include risks associated with the safety elements of working with the service user, their environment and any equipment in use that is required to provide the right support and to ensure that these are managed appropriately.

Our risk assessments will include challenging behaviour, self-harm risks assessments, skin integrity and pressure sores, mobility, nutrition and hydration, personal care risk assessment, housekeeping risk assessments and take into account other factors when allocating staff to work in people’s home. 

Support Plans

Support plan records will provide detailed information and guidance for staff on how to support people’s physical health needs and mitigate risk. For example, in a risk assessment for safer handling, we will identify the equipment in use; but we will also ensure that staff are trained on how to use the equipment and that staff are guided on how to support the person safely and in a way that encourages their independence. Staff will attend all mandatory training, including knowing how to identify the deteriorating Service User and how to access support for their health care needs.

General Wellbeing and Other Health Needs

We aim to provide a quality service where people living with other health needs such as depression, anxiety and other disorders to ensure they receive the right level of self-care and support they require to feel better and to greatly improve the quality of their lives. We aim to provide people with extremely positive experiences of the quality of the service and the care they receive, with support from staff who genuinely care and understand their needs.

We have systems and processes in place to keep people living with care and health needs, safe and protected from avoidable harm and to safeguard people from the risk of abuse. We do this by ensuring that we recruit staff with relevant skills, experience and knowledge to meet the needs of the people in our care. Our processes include safe recruitment procedures, ensuring that we recruit people with the relevant skills and background experiences, to be able to meet the needs of every individual.
