Our Service Types: Supported Living Services

We have defined our supporting living care packages below and in so doing, defined who we see as the potential service user for supported living care and support. Typically, the service user will be in a younger age bracket – aged 18-65 who we will support on a more permanent basis allowing for greater continuity of support provided. his could be a stepping stone for care leavers who wish to become more independent and eventually not depend on support. In our services, we envisage to assist with adults over the age of 65 and those under the age of 65, who need care and support in their own homes. Service users will be met through bespoke support plans tailored to their needs. Eligible adults will be those whose needs are assessed as best met through Supported Living care and support packages.

Potential service users are those who need rehabilitative assistance following hospital discharges which means initially people may need assistance with personal care, i.e., washing, bathing, showering, dressing, assistance to the toilet and intimate care to maintain their dignity , oral care, assistance with eating and drinking , skin care, support to access services for podiatry and other medical support needs or assistance with mobility. Our provision will be delivered within the persons own home/supported living accommodation and can be provided in any combination of care packages as detailed below.

Community Support

The person may require:

  • A few hours of support per week to develop/maintain their independence.
  • Focused on connecting and enabling and most aspirational in terms of enablement and promoting independence. The person may require supervision and prompting with personal care tasks and encouragement to achieve other activities of daily living.
  • The services required are similar to homecare, delivered as an outreach model to help people live independently in their own homes. This will be covered as an extension of our homecare business.

Daily care visits typically last for 30 minutes to an hour and can occur several times a day. Alternatively, you can opt to have visits known as ‘sits’ or ‘sitting’. These are longer visits that last several hours at a time. These ‘sits’ allow the care worker to assist with daily tasks, housework, prepare meals and take the person out for day trips, shopping or to local amenities and amusements. They are also trained to administer medication. Those who have ‘sits’ will generally be cared for by a rotation of two to three care workers who know them and their care plan inside out. This means that a deeper bond of friendship can be built, and care workers will find it easier to personalise daily activities.

There are different types of care packages, depending on the needs of the person needing care. We aim to support people requiring discharge from hospital who need assistance to regain their daily living skills and where possible reduce the amount of support they need.

Dedicated Supported Living Care

The person may require:

  • Standard Supported Living care package with variable day hours support from dedicated support workers.
  • Background support may not be required at all times; waking night support not usually required.
  • Due to combination of needs, there is a significant potential for support requirements to flex up and down to meet the service user’s changing needs;
  • The service user requires flexible shared hours that are not a barrier to new people moving into the service.

Service users are likely to require ongoing support, but  there also needs to be a focus on connecting and enabling people to maximise and promote their independence.

Standard Supported Living Care

The person may require:

  • Standard Supported Living care package, with variable day hours and night support according to their individually assessed need and packages of care.
  • Staff support is required, most of the time during the day and at night.
  • Support requirements may reduce and some flexibility is required around shared hours but unlikely to change significantly for the majority of service users in this service.
  • Care packages are enablement focused and the support required is likely to be 24 hours and more but support likely to be more consistent.

Intensive Supported Living Care

The person may require:

Enhanced/Intensive Supported Living care packages with 24/7 support

  • The service is designed for people who need Supported Living care packages, complete support all the time to achieve outcomes. Support requirements are unlikely to reduce significantly and more intensive, 1:1 support required with most aspects of daily living.
  • Focused on supporting in a way that minimises increases in need and enables people wherever possible.

Each service user will have their own tenancy agreement with their landlord, which will give them a home of their own to allow a greater level of autonomy as far as their environment is concerned and they will have a choice of who provides their support.  We will assist with empowering people to become independent by providing support with community engagement, further education, employment, and by achieving personal goals they have set.  Service users may become dependent on our staff for personal care for short periods of time, which would be more dignifying and less confusing for the service users.

Being registered with the CQC will eliminate the need to use agency staff during these periods. We are also aware that local councils require care providers to support service users in council owned properties but they require the care provider to be CQC registered before engaging and you have to be registered in order to bid/tender to provide the service.

In our Supported Living Services

We will provide individually tailored personal care and support to people in their own accommodation through our Supported Living packages of care. People will be responsible for their own tenancy agreements; Vivifica Healthcare Limited will supply the care and support to promote their independence. A personal support plan will be developed with each person using the service, identifying the areas where they need support to meet their individual needs and choices. We ensure that effective support is tailored to each individual as part of their personal support plan. Transport can be supplied to support them in being able to access the wider community and to be able to participate in their interests, hobbies, education and access to employment.

Within our Supported Living Service

  • We want to provide the right amount of care and support to help people achieve maximum possible independence and, by acquiring or re-acquiring basic living skills, to help them to achieve their full potential.
  • We aim to give people a greater individual say in how they live their lives and the services they need to help them to do so.
  • We want to promote choice and independence to all people who receive our services.

  • To provide our client group with an economical alternative to paying for assistance at home versus living in a larger, less personal facility, all whilst increasing the quality of life of the individual we support.
  • To use technology advancements which will facilitate easy access to information and effective communication.

We contribute to the community by providing our service users with specialised, well-trained professional Carers. Vivifica Healthcare Limited will provide our workforce with the necessary training needed to perform excellently when providing services in the community. We also intend to provide the right caliber of staff to meet the needs of our service users.

Vivifica Healthcare Limited aims to provide high quality care to the local population and beyond, meeting individual needs in a safe, caring, effective and responsive environment by a well-led workforce with the skills, knowledge and resources to meet the highest standards. Vivifica Healthcare Limited will train our staff to deliver support to people with a wide range of presentations and needs. With the increase in mental health issues, drugs and alcohol misuse in the UK population, especially in the areas where we are planning to operate, there is an increasing need for care providers to meet the growing specialist demand for care of adults over the age of 18.
